Transcripts Requests

The Registrar’s Office mails academic transcripts upon written authorization from the student. There is no fee charged for students currently attending class at Geisinger Commonwealth. A fee of $5 for each transcript requested is charged to students who are no longer attending or who have graduated. Transcripts are not released for students with financial holds. Although we attempt to process transcript orders within three working days of their receipt in our office, during peak times it may take longer. Please allow sufficient time for your request. To order transcripts by mail, please send your request to:

To order transcripts by fax, fax your request to:

The Geisinger Commonwealth’s Registrar’s Office at (570) 504-2799. Please mail a check or money order, if applicable, for $5 per transcript requested to the address listed above as soon as possible after your faxed request. Checks should be made payable to Geisinger Commonwealth School of Medicine.

Required Information:

  • Full name as listed on transcript. Include other names you used as a student at Geisinger Commonwealth
  • Empower ID (This can be found using Empower Web by clicking on “What’s my ID?”)
  • Date of birth
  • Dates of attendance and/or graduation
  • Degree received
  • Phone number where you can be reached if there are questions regarding your request
  • Your mailing address
  • Address where your transcript should be mailed
  • Number of transcripts requested
  • A check made payable to Geisinger Commonwealth School of Medicine, if applicable, for $5 per transcript requested
  • Most importantly, your legal signature. Due to the Family Educational Rights and Privacy Act (FERPA) of 1974, we cannot respond to a transcript request without your legal signature. Faxed signatures are permitted under law. The U.S. Department of Education has not yet ruled the recently enacted electronic signature legislation. Until such time as we are advised how this impacts FERPA legislation, we are unable to honor transcript requests by e-mail.
